Advanced Illness Management (AIM)

Advanced Illness Management is a program within Center for Primary Care that provides extra support to patients living with serious or long-term health conditions.

Our nurse practitioners work closely with the patient’s primary care physician to provide team-based, coordinated care. The program focuses on managing symptoms, preventing complications, and helping patients stay as healthy and comfortable as possible. AIM offers in-home added monitoring, education, and care planning to support both patients and their families while keeping the primary care provider at the center of care.
