We invite you to have greater access to CPC and your information through our secure Patient Portal. By taking a just a couple of minutes to register and log in, you are able to take advantage of the services listed below at any time. We want you to have access to this convenience, even after office hours. Get signed up by giving you CPC office your email address and then you are able to register online!

CPC’s PATIENT PORTAL ALLOWS YOU TO:

Review Personal Health Information (Lab results, Immunizations, Medications, Clinical Visit Summary)
Request prescription refills
Send a message to your doctor
Make a payment on your account
NEED AN EMAIL ADDRESS? You can register for a free email address at Gmail.com, Yahoo.com, or Hotmail.com
HOW TO SET UP A PATIENT PORTAL ACCOUNT:
  • Go to CenterForPrimaryCare.com
  • Click on the PATIENT PORTAL button at the top of the home page (This will take you to the secure patient portal site).
  • Click on the First Time Registration link on the right side of the page.
  • Complete the form: First & Last Name, email address (associated with your account), create password, and answer security questions. *Please note that you will be required to enter your Social Security number during registration. This helps ensure accurate linking of your medical records to your portal account.
  • Login to your account
