- Go to CenterForPrimaryCare.com
- Click on the PATIENT PORTAL button at the top of the home page (This will take you to the secure patient portal site).
- Click on the First Time Registration link on the right side of the page.
- Complete the form: First & Last Name, email address (associated with your account), create password, and answer security questions. *Please note that you will be required to enter your Social Security number during registration. This helps ensure accurate linking of your medical records to your portal account.
